Using the TM mark

How often do you use the TM mark in a manuscript? Is it each time the item that is trademarked appears or only the first time you use it?

According to the Chicogo Manual of Style, the symbol for TM or ® symbols need not be used in running text. Be sure to capitalize the registered trademark name, e.g., Kleenex, Xerox (but not photocopy).
